Step 3: After consultation, your SSM Academic Advisor must complete/sign the consortium form to confirm that the courses are valid for your SSM degree. Starting in the fall of 2015, UNT Dallas will operate independently of the UNT flagship. This independent operation and separation includes student enrolment, wire transfers, diplomas and transcripts. Consortium agreements allow students to enroll in courses at another institution (also known as a “host institution”) that are transferable to their curriculum at SSM. Consortium agreements allow a student to be registered simultaneously with both institutions, while benefiting from financial support from the SSM, based on the combined enrolment of both institutions. • Registration is limited to authorized courses. Your high school determines whether you have a double or a simultaneous loan. Only central courses are offered for alternating/ simultaneous registration. Collin students must complete the Collin/UNT Partnership Agreement Application Form prior to admission, available either on their campus or here on the Eagle Bound Program form, and submit the completed form to the UNT Office of Admissions.

With regard to consortium agreements at the SSM, the following provisions apply: • Students on double/concurrent credit are not entitled to development courses. 8. ET, you will also, at the end of the semester, submit an official academic transcript to the SSM Student Secretariat to confirm the completion of your courses; AND You must send the simultaneous registration application form (available under msutexas.edu/registrar/concurrent.php) to the SSM Student Secretariat prior to your enrolment with the SSM and your host institution. NOTE: If you do not provide the academic transcript, my notes/recordings will be saved until the transcript(s) “hold” is received. • Dual/concurrent high school students in their junior and senior years can take two courses per semester. In order to be able to take more than two courses, an official of the high school and university must give his consent (see page 2 of the registration form). How does the partnership contract work before authorization? 1. The enrolment period at the host institution shall coincide with the SSM enrolment period as follows: the UNT representative will contact the students and provide information on the services and resources made available to the participants in the Collin/UNT Partnership Agreement prior to admission. A student wishing to enroll simultaneously in another institution or university must have the written permission of his/her Dean/Academic Advisor of the UNT.

Transfer credits may be refused if this authorisation is not obtained in advance. If you have any general questions about the Dual Credit/Concurrent Enrollment program, please contact dualcredit@grayson.edu. Step 1: Fill in your student information and indicate the number of hours for which you are enrolled in each institution and indicate the host institution in which you will participate. NOTE: A consortium is only allowed if the hours recorded by the SSM are greater or equal to those registered at the HOST institution. students who are admitted to MSU and an accredited college/university, in accordance with specific enrollment agreements (including, but not only enrolled in, MSU Respiratory Care at Weatherford College and Collin College OR Radiology Program at North Central Texas College, Weatherford College, Tarrant County Community College and Brookhaven College or the RN-BSN Nursing Program at Tarrant County Community College or the VC2MSU Bridge Program), may however receive financial support to MSU, regardless of the number of hours spent at each institution. Also indicate the registration period for which you are requesting the consortium. ONLY CHECK ONE REGISTRATION PERIOD, as you must complete a separate consortium form for each registration period for which you are requesting the consortium….
